It can come from two places. The model repeats a wrong or outdated source, and it is fixed by fixing the source. Or it generates a plausible detail with no source, more common when search is off, and it is reduced with a clear, consistent brand entity on your website and official profiles.

It is documented like any other observation: question, platform, date, complete answer and, if there is one, the source. Each claim is classified as correct, partial, incorrect or not verifiable.
